Given its non-invasive nature, there is increasing interest in the use of transcutaneous vagus nerve stimulation across basic, translational and clinical research. Contemporaneously, tVNS can be achieved by stimulating either the auricular branch or the cervical bundle of the vagus nerve, referred to as transcutaneous auricular vagus nerve stimulation and transcutaneous cervical VNS, respectively. In order to advance the field in a systematic manner, studies using these technologies need to adequately report sufficient methodological detail to enable comparison of results between (...) studies, replication of studies, as well as enhancing study participant safety. We systematically reviewed the existing tVNS literature to evaluate current reporting practices. Based on this review, and consensus among participating authors, we propose a set of minimal reporting items to guide future tVNS studies. The suggested items address specific technical aspects of the device and stimulation parameters. We also cover general recommendations including inclusion and exclusion criteria for participants, outcome parameters and the detailed reporting of side effects. Furthermore, we review strategies used to identify the optimal stimulation parameters for a given research setting and summarize ongoing developments in animal research with potential implications for the application of tVNS in humans. Finally, we discuss the potential of tVNS in future research as well as the associated challenges across several disciplines in research and clinical practice. (shrink)

Maternal opioid use during pregnancy is a growing national problem and can lead to newborns developing neonatal opioid withdrawal syndrome soon after birth. Recent data demonstrates that nearly every 15 min a baby is born in the United States suffering from NOWS. The primary treatment for NOWS is opioid replacement therapy, commonly oral morphine, which has neurotoxic effects on the developing brain. There is an urgent need for non-opioid treatments for NOWS. Transcutaneous auricular neurostimulation, a novel and non-invasive form of (...) electrostimulation, may serve as a promising alternative to morphine. tAN is delivered via a multichannel earpiece electrode worn on and around the left ear, targeting two cranial nerves—the vagus and trigeminal nerves. Prior research suggests that auricular neurostimulation exerts an anxiolytic effect on the body by releasing endogenous opioids and reduces withdrawal symptoms in adults actively withdrawing from opioids. In this first-in-human prospective, open-label trial, we investigated tAN as an adjuvant to morphine therapy in eight infants >33 weeks gestational age suffering from NOWS and receiving oral morphine treatment. Infants received tAN for 30 min 1 h before receiving a morphine dose. tAN was delivered at 0.1 mA below perception intensity at two different nerve targets on the ear: Region 1, the auricular branch of the vagus nerve; and Region 2, the auriculotemporal nerve. tAN was delivered up to four times daily for a maximum of 12 days. The primary outcome measures were safety [heart rate monitoring, Neonatal Infant Pain Scale, and skin irritation] and morphine length of treatment. tAN was well-tolerated and resulted in no unanticipated adverse events. Comparing to the national average of 23 days, the average oral morphine LOT was 13.3 days and the average LOT after tAN initiation was 7 days. These preliminary data suggest that tAN is safe and may serve as a promising alternative adjuvant for treating NOWS and reducing the amount of time an infant receives oral morphine. (shrink)

Nishida Kitaro's reformulation of the major issues of Western philosophy from a Zen standpoint of "absolute nothingness" and "absolutely contradictory self-identity" represents the boldest speculative enterprise of modern Japan, continued today by his successors in the "Kyoto School" of philosophy. This English translation of Intuition and Reflection in Self-Consciousness evokes the movement and flavor of the original, clarifies its obscurities, and eliminates the repetitions. It sheds new light on the philosopher's career, revealing a long struggle with such thinkers as Cohen, (...) Natorp, Husserl, Fichte, and Bergson, that ended with Nishida's break from the basic ontological assumptions of the West. Throughout labyrinthine arguments, Nishida never loses sight of his theme: the irreducibility and unobjectifiability of the act of self-consciousness which constitutes the self. Extensive annotation is provided for the first time in any edition of Nishida's work. Historians of Japanese philosophy and culture, and all those interested in the interaction of Eastern and Western thought-forms, now have a document which highlights many of the cultural, psychological, and intellectual dynamics that have shaped Japanese intellectual life in one of its most fascinating and ambitious manifestations. Peter Abelard (1079–1142 ce) was the most wide‐ranging philosopher of the twelfth century. He quickly established himself as a leading teacher of logic in and near Paris shortly after 1100. After his affair with Heloise, and his subsequent castration, Abelard became a monk, but he returned to teaching in the Paris schools until 1140, when his work was condemned by a Church Council at Sens. His logical writings were based around discussion of the “Old Logic”: Porphyry's Isagoge, aristotle'S Categories and (...) On Interpretation and boethius'S textbook on topical inference. They comprise a freestanding Dialectica (“Logic”; probably c.1116), a set of commentaries (known as the Logica [Ingredientibus], c. 1119) and a later (c. 1125) commentary on the Isagoge (Logica Nostrorum Petititoni Sociorum or Glossulae). In a work Abelard called his Theologia, issued in three main versions (between 1120 and c.1134), he attempted a logical analysis of trinitarian relations and explored the philosophical problems surrounding God's claims to omnipotence and omniscience. The Collationes (“Debates,” also known as “Dialogue between a Christian, a Philosopher and a Jew”; probably c.1130) present a rational investigation into the nature of the highest good, in which the Christian and the Philosopher (who seems to be modeled on a philosopher of pagan antiquity) are remarkably in agreement. The unfinished Scito teipsum (“Know thyself,” also known as the “Ethics”; c.1138) analyses moral action. (shrink)

Apocalyptic expectations of Armageddon and a New Age have been a fixture of the American cultural landscape for centuries. With the approach of the year 2000, such millennial visions seem once again to be increasing in popularity. Stephen O'Leary sheds new light on the age-old phenomenon of the End of the Age by proposing a rhetorical explanation for the appeal of millennialism. Using examples of apocalyptic argument from ancient to modern times, O'Leary identifies the recurring patterns in apocalyptic texts and (...) movements and shows how and why the Christian Apocalypse has been used to support a variety of political stances and programs. The book concludes with a critical review of the recent appearances of doomsday scenarios in our politics and culture, and a meditation on the significance of the Apocalypse in the nuclear age. Arguing the Apocalypse is the most thorough examination of its subject to date: a study of a neglected chapter of our religious and cultural history, a guide to the politics of Armageddon, and a map of millennial consciousness. Of recent time, there has been a proliferation of concerns with ethical leadership within corporate business not least because of the numerous scandals at Enron, Worldcom, Parmalat, and two major Irish banks – Allied Irish Bank (AIB) and National Irish Bank (NIB). These have not only threatened the position of many senior corporate managers but also the financial survival of some of the companies over which they preside. Some authors have attributed these scandals to the pre-eminence of a focus on (...) increasing shareholder value in Western business schools and/or to their failure to inculcate ethical standards. In this paper, we challenge these accounts and the aetiological view of knowledge from which they derive but are grateful for the consensus that they convey regarding the importance of business ethics. The paper focuses on different approaches to ethical leadership concluding with a view that some hybrid of MacIntyre’s virtue ethics and Levinas’s ethics of responsibility may serve as an inspiration for both educators and practitioners. Biomedical diagnostic science is a great deal less successful than we've been willing to acknowledge in bioethics, and this fact has far-reaching ethical implications. In this article I consider the surprising prevalence of medically unexplained symptoms, and the term's ambiguous meaning. Then I frame central questions that remain answered in this context with respect to informed consent, autonomy, and truth-telling. Finally, I show that while considerable attention in this area is given to making sure not to provide biological care to (...) patients without a need, comparatively little is given to the competing, ethically central task of making sure never to obstruct access to biological care for those with diagnostically confusing biological conditions. I suggest this problem arises from confusion about the philosophical value of vagueness when it comes to the line between biological and psychosocial needs. (shrink)

The strongest version of the principle of the Identity of Indiscernibles states that of necessity, there are no distinct things with all their universals in common (where such putative haecceities as being Aristotle do not count as universals: I use 'universal' rather than 'property' here and in what follows for the simple reason that 'universal' is the term of art that most safely excludes haecceities from its instances). It is commonly supposed that Max Black's famous paper 'The identity of indiscernibles' (...) (2) refutes this thesis. (Armstong's [1], chapter 9 is representative here.) Black argues ([2], p. 156) that it is perfectly possible that there be a world consisting solely of two indiscernible spheres at some distance to each other and that this world constitutes a counterexample to the principle above. The strongest version of the bundle theory of substance claims that of necessity, the substances that make up the world are bundles of universals.1 It is commonly supposed that a consequence of Black's defeat of the principle of the Identity of Indiscernibles is that this bundle theory of substance is mistaken. (Again, Armstong's [1] is representative.) I shall argue that Black's thought experiment does not defeat the bundle theory and that, as a result, the bundle theory can be used to salvage the principle of the Identity of Indiscernibles. This book is of interest to those working at the intersection of contemporary debates in philosophy, ethics, politics and cultural studies.

This paper discusses three cultural narratives that threaten the health of people with intellectual/developmental disabilities (IDD) and which have become more evident during the COVID-19 pandemic. These meta-narratives are the medical model of health/disability; the population health approach to health inequalities; and policies premised on the assumption of the importance of national economic growth as an incentive for reducing health inequalities. Evidence exists that health research is more likely to become policy if it fits within a medical model and addresses (...) national economic growth. These two criteria are particularly problematic for people who have IDD. The paper also proposes a research model to facilitate the inclusion of IDD related issues in future policy regarding health inequalities. (shrink)

The ArgumentThis paper argues that science and technology studies need to adopt a much wider view of what counts as a laboratory. The factory, it is suggested, is as much a site of invention and intervention as the laboratory. As a site for the government of economic life, the factory is a laboratorypar excellence. One particular factory is studied — the Decatur, Illinois, plant of Caterpillar Inc. — as it is rethought and remade in accordance with ideals of cellular manufacturing, (...) Just-In-Time systems, customer-driven manufacturing, and competitor benchmarking. But it is not just the changes at the factory itself that are studied. The paper analyzes the linkages and relays between the redesign of a particular manufacturing plant and the plethora of calls for a revitalization of North American manufacturing industry and a new form of economic citizenship. The paper examines the remaking of a factory as anassemblage, a historically specific and temporarily stabilized complex of relations among ways of problematizing the factory in a multiplicity of locales. There are four steps to the changes analyzed here: a problematizing of the factory at the level of North American manufacturing as a whole in the 1980s; a problematizing of the notion of competitiveness at Caterpillar Inc, through the calculative practices of competitor benchmarking and related expertises; a diagraming of the ideal factory in systems terms; and the embedding of notions of the product, of competitiveness, and of a new economic citizenship in the “Assembly Highway” at the Decatur plant. Rethinking the factory took place within this assemblage of relations, rether than at any one site. (shrink)

According to an externalist theory of content the content of an individual’s thoughts and the meaning of her words need not supervene on her intrinsic history. Two individuals may be intrinsically exactly alike yet entertain different thoughts, and attach different meanings to the words they use. ETC, which has been most notably defended by Saul Kripke, Hilary Putnam and Tyler Burge, has attained the status of current orthodoxy. Nevertheless, some maintain that combining ETC with the premisses that we have privileged (...) access to our intentional states yields a surprising conclusion. It yields the conclusion that we can have a priori knowledge of features of the external world that seem only accessible to empirical investigation. Call the argument from ETC and privileged access to this conclusion the main argument. Some take the main argument to be a reductio of ETC. Since we have privileged access to our intentional states, and lack the a priori knowledge attributed by the main argument, ETC is false. Some take the main argument to refute the premisses that we have privileged access to those intentional states falling within the scope of ETC. Others see the main argument as a refutation of external world scepticism. We will show that the main argument does not support any of these interesting conclusions. We will show that the main argument does not threaten to restrict privileged access, refute external world scepticism, or constitute a reductio of ETC. More exactly, we will show that the most defensible version of ETC, even when combined with the premiss that we have unrestricted privileged access to our intentional states, yields no a priori knowledge of the external world. There is a class of fictionalist strategies (the reflexive fictionalisms) which appear to suffer from a common problem: the problem that the entities which are supposedly fictional turn out, by the lights of the fictionalist theory itself, to exist. The appropriate solution is to reject so-called strong fictionalism in each case: that is, to reject the variety of fictionalism which takes appeal to the domain of fictional entities to provide an explanation or analysis of the operators or predicates with which (...) the objects are systematically correlated. This paper canvasses the various dimensions along which theories of truth may disagree about the extent to which truth is minimal.

We generally accept that medicine’s conceptual and ethical foundations are grounded in recognition of personhood. With patients in vegetative state, however, we’ve understood that the ethical implications of phenomenal consciousness are distinct from those of personhood. This suggests a need to reconsider medicine’s foundations. What is the role for recognition of consciousness (rather than personhood) in grounding the moral value of medicine and the specific demands of clinical ethics? I suggest that, according to holism, the moral value of medicine is (...) secured when conscious states are recognized in everyday medical science. Moreover, consciousness fully motivates traditional principles of clinical ethics if we understand respect for autonomy as respect for the dominion of an experiencer in the private, inescapable realm of bodily experience. When medicine’s foundations are grounded in recognition of consciousness, we understand how patients fully command respect even when they lack capacity to exercise their bodily dominion through decision-making. This paper provides a detailed study of fraud in practice through an empirical investigation of B.P.Sayers, a family-owned stockbroking firm that had been in existence for over 100 years and that collapsed due to the fraudulent activities of the firm's junior partner. An interpretive narrative methodology has been employed which has resulted in the development of a detailed understanding of fraud and moral breakdown in organizations, resulting from a failure of responsibility that arises from a dysfunctional work identification and its (...) moral implications. In developing this account of fraud, the conceptual framework utilized is drawn from the moral philosophical work of Emmanuel Levinas who explicated how the breakdown of social relations of responsibility results, in turn, in moral breakdown. Ethical instruction is critical for trainee accountants. Various teaching methods, both active and passive, are normally utilised when teaching accounting ethics. However, students’ learning styles are rarely assessed. This study evaluates the learning styles of accounting students and assesses the interaction of teaching methods and learning styles in an ethics instruction environment. The ethical attitudes and preferred learning styles of a cohort (137) of final year accounting students were evaluated pre-instruction. They were then subject to three different teaching methods while (...) studying ethics during an auditing course. When ethical attitudes and preferred learning styles were re-assessed post-instruction, the teaching methods were found to have influenced active learners more than passive ones. Furthermore, when learning styles matched teaching methods used, usefulness was assessed as high but when learning styles and teaching methods differed, usefulness deteriorated significantly. Students displayed a preference for passive learning styles, despite being so advanced in their education. The implications are that instructors should consider learning styles before deciding on appropriate teaching methods, in accounting ethics environments. (shrink)

Empirical evidence advancing the theory of anthropogenic climate change and resultant policy action has been framed through the perspectives of scientists, economists and politicians; the ultimate objective being to minimise the risk of dangerous climate change through the reduction of GHG emissions. However, policies designed to reduce carbon pollution have utilised cost benefit analysis , largely ignoring ethical implications of such actions. This has resulted in a climate debate that sidelines the moral and social considerations of the suggested actions designed (...) to reduce the impact of dangerous climate change. Special interest groups have focussed on subjective interpretations of objective statistics, resulting in an extreme polarisation of viewpoints, pitting climate change deniers against politico-economic rationalists, with climate scientists largely sitting on the sidelines. There has, however, been a recent shift in the science of climate change acknowledging that, “many profound ethical questions are hidden in scientific and economic arguments about various climate change policy proposals” ‘White Paper on the Ethical Dimensions of Climate Change’, Rock Ethics Institute, Penn State University). We wish to promote an inter-disciplinary approach to the ethical considerations of climate change and situate the debate within a ‘real world’ context to enable people to link evidence and resultant policy to their own moral responsibility as ‘global citizens’. By placing reliable scientific information within a holistic framework, scientific outcomes with a high level of certainty may be conveyed within a wider perspective that includes ethical and social considerations, resulting in a stronger claim for practical outcomes with respect to climate change solutions. This can be used to support policies that are practical as well as moral. (shrink)
